Ask for the Wellness Claim Department
Have the following information available:
1. Date of service performed
2. Type of service performed (annual physical, annual eye exam, or annual dental exam
If this was an Annual doctors visit with a full physical & blood work, you will receive 2 separate checks for $100.00 each (Hospital Indemnity & Accident Wellness)
If you file an Annual doctors visit for Dental, Vision, or a Mammogram, you will need the same information to give to them as listed above for a wellness claim but you will only receive 1 check for $100.00.
